Description

Introduction
G Gentamicine is an antibiotic instilled in the eye to treat bacterial infections of the eye (eg. conjunctivitis). Additionally, it may be instilled in the ear to treat bacterial infections of the ear (eg. otitis externa). G Gentamicine also helps to prevent an infection after eye/ear injury or surgery. It fights the infection by stopping the growth of bacteria. This helps to resolve your symptoms and cure the underlying infection. Follow your doctor’s instructions precisely for administering this medicine in the eye/ear. Wait for at least 5 minutes before applying any other medication to the same eye/ear. It should be used regularly at evenly spaced time intervals as prescribed by your doctor. Do not skip any doses and finish the full course of treatment even if you feel better. Stopping the medicine too early may lead to the infection returning or worsening. It is a broad-spectrum type of antibiotic effective in killing many types of bacteria. However, it will not work for other types of eye/ear infections (e.g. viral) and therefore, should only be used if prescribed by your doctor. Unnecessary use of any antibiotic can lead to a decrease in its effectiveness in the future. This medicine may cause some side effects like temporary eye/ear irritation and burning sensation after administration. Consult your doctor if these side effects persist or if your condition worsens. It may cause some short-term blurring of vision. Therefore, do not drive immediately after administering this medicine in the eye. Contact lenses should not be worn while you are using this medicine for an eye infection.

How to use Nicogenta Eye/Ear Drops
This medicine is for external use only.Take it in the dose and duration as advised by your doctor. Check the label for directions before use. Hold the dropper close to the eye/ear without touching it. Gently squeeze the dropper and place the medicine inside the lower eyelid or ear. Wipe off extra liquid.

Do not touch the tip to any surface, or to your eye/ear, to avoid contamination.
Wait for at least 5-10 minutes before delivering the next medication in the same eye/ear to avoid dilution.

Adult Dose
Ophthalmic Superficial ophthalmic infections Adult: As eye drops: Instill 1-2 drops into affected eye up to 6 times daily. For severe infections, initially, 1-2 drops every 15 min, reducing the frequency gradually as the infection is controlled. As eye oint: Place a small amount into the conjunctival sac 2-3 times daily. Otic/Aural Otitis externa Adult: As ear drops: Instill 2-3 drops in the affected ear 3-4 times a day and at night.

Contraindication
History of hypersensitivity to aminoglycoside; pregnancy; hepatic impairment, perforated ear drum.
Mode of Action
Gentamicin is an aminoglycoside that binds to 30s and 50s ribosomal subunits of susceptible bacteria disrupting protein synthesis, thus rendering the bacterial cell membrane defective.
Precaution
Concurrent use of neuromuscular blocking agents; myasthenia gravis, parkinsonism; conditions predisposing to ototoxicity and nephrotoxicity; lactation. Monitor plasma concentrations of gentamicin in patients receiving high doses or prolonged courses, in infants, elderly, patients with renal impairment, cystic fibrosis or significant obesity. Monitor auditory and renal functions. Lactation: safety in nursing infants unknown

Interaction
Additive effect w/ other neurotoxic and/or nephrotoxic drugs including cephalosporins, methicillin, amphotericin B, ciclosporin, cisplatin, potent diuretics (e.g. ethacrynic acid, furosemide) and neuromuscular blocking agents (e.g. succinylcholine, tubocurarine). May potentiate the effect of anticoagulants (e.g. warfarin, phenindione). May antagonise the effect of neostigmine and pyridostigmine. Increased risk of hypocalcaemia w/ bisphosphonates. Increased risk of neuromuscular blockade w/ botulinum toxin. Indometacin may increase the plasma concentration of gentamicin in neonates.
